The name Reaper is derived from the fact that they reap Grounders. They go out and collect Grounders and bring them to Mount Weather, who needs them. The people in Mount Weather can't survive on.

The reapers are crazy cannibals who kill people mercilessly and seem to lack all humanity. Yet, they also respond to that one particular tone and cower in fear. They're obviously training Lincoln, via a Pavlov's Dog type conditioning, to cower in fear when he hears the same tone.

They showed them how to "heal" the reapers as part of an agreement. So they are normal grounders and then probably died in Praimfiya. IIRC, the original agreement was that Skaikru (Abby) was going to help restore them to their pre-reaper state. Once Lexa broke their pact, that probably never happened, so - most of them probably died.

They live in the tunnels beneath Mount Weather, and are created in a process called Project Cerberus that uses repeated injections of a red drug. There has never been a successful reversal of a Project Cerberus subject.

There were several types of Reapers, Lincoln physical prowess marked him for cerberus project, a guard dog. I'm pretty sure all the reapers were part of the Cerberus project, because all the captives were divided between 'harvest' and 'Cerberus'. And it seemed pretty hard to break the addiction, they had to chain him down and he almost died.
